Terms and Conditions

Introduction and Acceptance

These Terms and Conditions are an agreement between you and Ozone
Wellness LLC, doing business as SignalRx (“SignalRx,”
“we,” “us,” or “our”), the
operator of signal-rx.com. They govern your Signal-RX account, online
intake and health-update forms, provider-review services, medication
ordering, and related support services (collectively, the
“Services”).

By using the website, creating an account, submitting an intake or
health update, paying a provider-review fee, checking an acceptance
box when presented, or placing an order, you agree to these Terms. If
you do not agree, do not use the Services.

Signal-RX, Clinicians, and Pharmacies

Signal-RX provides technology, administrative, e-commerce,
care-coordination, and fulfillment-support services. Participating
licensed clinicians and clinical entities provide clinical services
and make all decisions concerning eligibility, diagnosis,
prescribing, treatment approval, renewal, and discontinuation.
Participating dispensing pharmacies fill and ship prescriptions.
Signal-RX does not itself prescribe or dispense medication.

The professional relationship for clinical care is between you and
the participating clinician or clinical entity that accepts your
case. That relationship may be governed by additional consents,
notices, and professional obligations.

Telehealth and Emergency Limitations

Signal-RX generally uses an asynchronous review process. An
appointment or video call is not ordinarily required, although a
clinician may request additional information or determine that
another form of evaluation is necessary. You consent to the use of
electronic communications and records for telehealth as described in
the consent presented during the intake process.

The Services are not intended for emergencies or urgent medical
problems. Call 911 or seek immediate in-person emergency care if you
believe you are experiencing a medical emergency. Do not wait for an
email, portal response, provider review, or order update.

Eligibility

To use the clinical and medication-ordering Services, you must:

  • Be at least 18 years old and legally able to enter an agreement.
  • Provide accurate identity, contact, location, health, and payment
    information.
  • Be physically located in, and where required be a resident of, a
    jurisdiction where the applicable clinician may provide care and
    the requested treatment is available.
  • Use the Services only for yourself and for lawful purposes.

Signal-RX currently supports eligible patients in Florida,
Massachusetts, New York, Oregon, and Pennsylvania. Availability may
change and may vary by clinician, treatment, pharmacy, and applicable
law. The eligibility information shown during intake and checkout
controls when a request is submitted. You must accurately report your
physical location when submitting an intake, health update, or other
clinical request.

Identity Verification

By accepting these Terms and the Privacy Policy and paying for an
eligible telehealth consultation, you authorize Signal-RX to begin the
current automatic identity-verification sequence after verified
payment and, for a newly created account, after secure account setup
is complete. No additional general identity-verification checkbox is
presented at checkout. Identity verification is an account,
patient-safety, fraud, and compliance measure; it does not make a
clinical, prescribing, treatment, or medication-approval decision.

Signal-RX uses Vouched Identity, Inc. (“Vouched”) for this
server-to-server sequence. Signal-RX first performs Vouched
CrossCheck. If CrossCheck returns a definite nonmatch, Signal-RX
performs one date-of-birth comparison. If either check verifies the
patient, the automatic identity-verification sequence is complete. If
the second check does not verify the patient, or if an automatic
check has a technical, ambiguous, malformed, timeout, or other
operational failure, the automatic sequence ends and the patient is
directed to contact support.

For these checks, you authorize Signal-RX to provide Vouched with the
patient’s name, date of birth, email address, telephone number,
home address, and the Internet Protocol address associated with the
consultation checkout for identity-verification and fraud-prevention
purposes. Additional details about collection, timing, operational
retention, and deletion appear in our
Privacy Policy.

Under the current policy, Signal-RX does not ask a patient to upload
a government-issued identification document, selfie, facial image, or
biometric data for automatic identity verification and does not
start, resume, or retry a photo-ID or Visual verification session.
Historical Visual jobs invoked before August 16, 2026 remain subject
to the contained callback, reconciliation, support, deletion, and
retention practices described in the Privacy Policy; they do not
authorize or unlock a new capture.

You agree to provide accurate identity information. If automatic
identity verification does not complete, follow the Contact Support
instructions shown on the website. Do not email medical information,
documents, selfies, or identification images to support.

Account Responsibilities

You are responsible for maintaining the confidentiality of your login
credentials and for activity performed through your account. Do not
share your account or allow another person to submit health
information, request treatment, or order medication in your name.

Keep your contact, address, and account information current. Notify
support@signal-rx.com
promptly if you suspect unauthorized access or believe account
information is inaccurate.

Accurate Health Information

You must provide truthful, accurate, current, and complete health
information, including medications, allergies, diagnoses, symptoms,
reproductive status, and other facts requested by the clinician.
Deliberately false, misleading, omitted, or outdated information may
endanger your health and may result in a request for more information,
nonapproval, suspension, or termination of access.

You are responsible for reporting material health changes and for
completing required health updates. Website content and general
treatment descriptions are educational only and are not individualized
medical advice.

Provider Review and Prescribing Decisions

Submitting an intake, paying a provider-review fee, selecting a
treatment, or placing an order does not guarantee approval,
prescribing, or fulfillment. A licensed professional makes each
clinical decision using professional judgment. Automated systems do
not approve or deny treatment.

A clinician may approve one or more requested treatments, decline a
request, request additional information, recommend another form of
care, or discontinue treatment. The clinician has no obligation to
prescribe a requested medication.

Approval Periods and Health Updates

Provider approval is limited to the treatments and products
specifically approved for you and remains subject to the approval
period shown in your account. Continued prescribing may require a new
health questionnaire and another provider decision, generally at
six-month intervals.

Approval may expire, be paused, not be renewed, or be stopped by a
clinician. Ordering is unavailable when current approval is absent or
expired. There is no automatic clinical approval, renewal, refill, or
medication shipment.

Provider-Review Fee

Signal-RX charges a one-time $5 provider-review fee, paid once, for
review of one or more requested treatments. The fee is separate from
medication, shipping, and tax charges and is non-refundable once paid.
No additional provider-review fee is charged for a required six-month
health update or an ordinary medication order.

The fee remains non-refundable when a clinician requests more
information, determines that treatment is not appropriate, or does
not issue a prescription. Paying the fee does not purchase a
prescription or guarantee a clinical outcome.

Medication Prices, Shipping, and Payment

Medication prices, applicable taxes, and shipping are displayed
before payment. Medication prices do not include the separate
provider-review fee. Signal-RX currently charges one flat $20
medication-shipping charge per medication order, regardless of the
number of treatments, quantities, packages, or dispensing pharmacies.

Prices and availability may change before an order is submitted. A
medication order retains the price accepted at payment. Before
provider approval, Signal-RX may correct or cancel an order affected
by an obvious pricing error, fraud concern, or legal restriction. If a
paid order is canceled before provider approval, the affected
medication charge, applicable tax, and shipping charge will be
returned to the original payment method. Once a provider approves the
order, the cancellation rule in the Medication Orders section applies.

Payments are processed by third-party payment processors and gateway
providers. You authorize the applicable processor to charge the
payment method you submit for the amounts shown at checkout. Signal-RX
offers no membership or subscription and does not automatically charge
for refills or medication shipments.

Medication Orders

Each medication order is subject to provider review even when you
have an active treatment approval. Payment and order submission do
not guarantee fulfillment. The provider may approve or not approve
the medication order based on current clinical information.

If a provider does not approve a medication order, the medication
charges, applicable tax, and associated shipping charge are refundable
to the original payment method. The separate $5 provider-review fee
remains non-refundable.

You may request cancellation only before provider approval of the
medication order. Once approved, the order cannot be canceled,
changed, or redirected, even if pharmacy preparation or shipment has
not begun. Verified damaged or incorrect products remain eligible for
the remedies described in the
Refund Policy.

Prescription and Compounded Medications

Prescription medication requires a valid prescription from a licensed
clinician and is dispensed by a participating pharmacy. You agree to
use medication only as directed, follow its label and storage
instructions, never share, transfer, or resell it, and report adverse
reactions or concerns promptly to your clinician.

Some prescribed medications may be compounded. Compounded drugs are
not FDA-approved, and the FDA does not review their safety,
effectiveness, or quality before they are marketed. Compounding and
dispensing are performed by pharmacies operating under the federal
and state requirements applicable to them. A clinician determines
whether a particular treatment is medically appropriate.

A clinician may also prescribe an FDA-approved medication for a use
not included in its approved labeling when the clinician determines
that the use is medically appropriate and permitted by law. Such
“off-label” prescribing does not mean that the particular
use has been approved by the FDA.

Prescription medications cannot be returned or restocked after
dispensing. Exceptions for verified damaged or incorrect products are
described in the Refund Policy.

Pharmacy Fulfillment and Shipping

An approved order may be sent to one or more dispensing pharmacies
and may arrive in separate packages with separate tracking notices.
Separate packages do not create additional Signal-RX shipping charges.

Pharmacies process approved prescriptions on business days. Orders
approved Friday through Sunday generally begin pharmacy processing
Monday, or the next business day when Monday is a holiday. Beginning
processing does not guarantee shipment that day. Processing,
shipment, carrier transit, and delivery dates are estimates.

You are responsible for providing an accurate eligible shipping
address. Additional requirements, delivery rules, lost or damaged
package procedures, and timing information appear in the
Shipping Policy.

Electronic Communications and Signatures

You consent to receive electronic records and neutral operational
communications concerning your account, review status, orders,
payment, dispatch, and tracking. Communications may be delivered by
email, through your account, or through the secure patient portal.
You are responsible for maintaining a working email address and
reviewing account notices.

Electronic actions, typed names, checked attestations, and other
accepted electronic methods may serve as your signature to the extent
permitted by law. Do not send medical details through ordinary email.
Use the secure patient portal for clinical questions and sensitive
health information.

Privacy and Clinical Records

The Privacy Policy explains how
Signal-RX collects, uses, and shares information for the website and
Services. HIPAA-regulated clinical information may also be governed by
the participating clinical entity’s separate Notice of Privacy
Practices, when applicable. That notice is made available through the
applicable clinical service or patient process.

If these Terms conflict with mandatory privacy rights, professional
duties, or applicable law, those controlling requirements apply.
Suspending a website account does not eliminate rights concerning
access to medical records.

Intellectual Property

The website, branding, text, graphics, images, page layouts, software,
and other content are owned by or licensed to Signal-RX and are
protected by applicable intellectual-property laws. Subject to these
Terms, Signal-RX grants you a limited, revocable, nonexclusive,
nontransferable right to use the website for your personal,
noncommercial use.

You may not copy, reproduce, distribute, sell, modify, publicly
display, reverse engineer, or create derivative works from protected
website content except as permitted by law or with prior written
authorization.

Prohibited Conduct

When using the Services, you may not:

  • Provide deliberately false, misleading, or fraudulent information.
  • Impersonate another person or use another person’s account.
  • Obtain, share, transfer, or resell prescription medication unlawfully.
  • Access or attempt to access accounts, records, systems, or data
    without authorization.
  • Introduce malicious code, disrupt the Services, evade safeguards,
    or interfere with other users.
  • Scrape, crawl, harvest, or use automated tools against the website
    without written permission, except as permitted by law.
  • Use the Services in violation of law or another person’s rights.

Third-Party Services and Links

The Services rely on third parties, including clinicians, clinical
entities, pharmacies, payment processors, hosting and security
providers, email providers, and shipping carriers. Their services may
be governed by separate terms and privacy notices.

The website may link to third-party sites that Signal-RX does not own
or control. Signal-RX is not responsible for third-party content,
availability, security, or privacy practices merely because a link is
provided.

Disclaimer of Warranties

Except as expressly stated or required by law, the website,
technology, and general informational content are provided on an
“as available” basis. Signal-RX does not guarantee
uninterrupted or error-free website operation, a prescription, a
particular treatment result, pharmacy processing time, carrier
performance, or a delivery date.

These Terms do not replace a clinician’s professional duties, a
pharmacy’s legal obligations, product warranties that cannot be
disclaimed, or rights and remedies that cannot lawfully be limited.

Limitation of Liability

To the fullest extent permitted by law, Signal-RX will not be liable
for indirect, incidental, special, consequential, exemplary, or
punitive damages arising from use of, or inability to use, the
nonclinical website and platform Services.

Nothing in these Terms excludes or limits liability for fraud, willful
misconduct, gross negligence, personal injury, statutory consumer
rights, or any other liability that cannot legally be excluded or
limited. These Terms do not eliminate the independent professional
duties of a participating clinician, clinical entity, or dispensing
pharmacy.

Indemnification

To the extent permitted by law, you agree to indemnify and hold
Signal-RX harmless from third-party claims, losses, and reasonable
costs caused by your unlawful use of the Services, intentional
submission of materially false information, infringement of another
person’s rights, or material breach of these Terms.

This obligation does not require you to indemnify any person or entity
for its own negligence, professional misconduct, unlawful conduct, or
other responsibility that cannot legally be shifted to you.

Suspension and Termination

Signal-RX may restrict or suspend website or account access when
reasonably necessary to address fraud, misuse, a security risk,
deliberately inaccurate information, nonpayment, unlawful activity,
or a material violation of these Terms, subject to applicable law.

Suspension does not eliminate payment obligations already incurred,
rights concerning medical records, or professional obligations that
apply to a participating clinician or clinical entity. Provisions that
by their nature should survive termination will remain effective,
including payment, intellectual-property, disclaimer, liability,
indemnification, and governing-law provisions.

Governing Law

These Terms are governed by the laws of the State of Florida, without
regard to conflict-of-law principles, except where the mandatory law
of another jurisdiction applies. To the extent legally permitted,
proceedings concerning the nonclinical website or platform Services
will be brought in an appropriate state or federal court serving
Sarasota County, Florida.

Nothing in this section prevents you from using a small-claims process
or exercising a venue, limitation period, or other right that cannot
lawfully be waived.

Changes to These Terms

Signal-RX may update these Terms as the Services or applicable
requirements change. The revised version will identify a new
effective date. Notice of material changes will be provided when
required by law.

Changes apply prospectively and do not alter the accepted price or
terms of an order already completed unless permitted by law. Continued
use after revised Terms become effective constitutes acceptance of the
revised Terms where legally permitted.

General Terms

These Terms, together with the
Privacy Policy,
Refund Policy,
Shipping Policy, and any consent or
terms presented for a particular Service, form the agreement governing
your use of the Services.

If a provision is found invalid or unenforceable, it will be modified
only to the minimum extent necessary and the remaining provisions will
continue in effect. A failure to enforce a provision is not a waiver.
You may not assign your rights under these Terms without written
consent. Signal-RX may assign these Terms in connection with a lawful
reorganization, financing, merger, or transfer of the Services,
subject to applicable law.
